Relays will only be entered if all 5 swimmers are available & swimmers notify Damien via email or SMS by 5pm Monday 27 January 2014.

ENTRIES:
PLC Aquatic will enter you in the swim meet and invoice you the entry fees.

Swimmers must notify Damien of the events they wish to enter by 5pm Monday 27 January 2014.

UNIFORM:
This season PLC Aquatic will provide swimmers with 1 State Team Tee-shirt when they first qualify for the Victorian Championships. Swimmers who require a 2nd tee shirt can purchase them.
If you have qualified for the 1st time with PLC Aquatic (Those that don’t already have a Victorian Championships Tee-shirt) MUST FILL OUT THE FOLLOWING FORM AND RETURN IT TO DAMIEN BY 5pm Monday 27 January 2014.

Swimmers with ** next to their name will receive a free tee-shirt provided their form is returned by the due date.
